Is HIFU right for me? A guide for men exploring prostate cancer treatment

Being told you have prostate cancer can feel incredibly overwhelming. Alongside processing the diagnosis itself, you’re suddenly faced with decisions about surgery, radiotherapy, treatment options and, depending on your consultant's specialist area, perhaps something called HIFU.

So how do you know if high intensity focused ultrasound (HIFU) is the right option for you?

The answer depends on several important clinical factors and just as importantly, your personal priorities for your care.

What makes someone suitable for HIFU?

HIFU is most commonly used for men with localised prostate cancer, meaning the cancer is confined to the prostate and has not spread elsewhere in the body.

You may be suitable if:

• Your cancer is visible on MRI – so it can be clearly targeted

• It is confined to one area (or one side) of the prostate – it hasn't spread outside the gland

• Your Gleason score and PSA levels fall within an appropriate range (typically a Gleason score of 7 or lower)

• You are looking for a focal therapy approach rather than full gland removal (known as a radical prostatectomy)

It's important to remember that every case is different. Your specialist will carefully review your MRI scans and biopsy results before making any decision on your care.

When HIFU may not be recommended

It's important to remember that HIFU is not suitable for everyone and it may not be appropriate if:

• The cancer has spread outside the prostate (it has metastasised)

• There are multiple aggressive areas throughout the gland (it's no longer localised)

• Imaging does not clearly define the tumour location

• Your cancer risk category suggests surgery or radiotherapy would be a more effective course of treatment

The goal is always the same: choosing the treatment that offers the best long-term cancer control for you.

Why some men choose HIFU

HIFU treatment offers potential advantages for men with prostate cancer:

• Treatment is targeted to the cancerous tissue

• It preserves healthy prostate tissue

• There is a reduced risk of urinary incontinence when compared with radical prostatectomy

• There may be a lower risk of erectile dysfunction in carefully selected cases

• Recovery time can be shorter than with more invasive treatments

For many men, the advantages of HIFU lie in treating the cancer in a targeted way while aiming to protect quality of life, something that matters greatly when weighing up treatment options.

HIFU vs Surgery vs Radiotherapy

Understanding your treatment options is incredibly important if you’re faced with multiple choices — and this is something your consultant should discuss in detail with you. As an overview::

• Surgery removes the entire prostate – including healthy tissue

• Radiotherapy treats the whole gland with radiation – including healthy tissue

• HIFU, when used as focal therapy, treats only the cancerous area.

This difference matters.

However, it’s important to understand that long-term data, cancer characteristics, and individual anatomy all influence which treatment is safest and most effective. There is no “one size fits all” answer – only what is right for you, as the patient.

So, Is HIFU Right for You?

For the right patient, HIFU can be an excellent treatment option. For others, surgery, radiotherapy or chemotherapy may be safer and more effective.

• Is my prostate cancer localised and clearly visible on MRI?

• Am I clinically suitable for focal therapy such as HIFU?

• What impact might each treatment have on urinary continence and sexual function?
